Your first visit takes about 45 to 60 minutes. Arrive 15 to 20 minutes early and bring a photo ID, your insurance card, a list of your medications, and any MRI, X-ray or CT images you have. If you're coming in under workers' compensation or a no-fault auto claim, bring the claim details as well.

We're a board-certified interventional pain practice at 9701 66th Ave in Rego Park, and we've treated patients across Queens since 2004. Walking into a new doctor's office while you're hurting is stressful enough, so this page covers what happens, what to bring, and how insurance works. That way there are fewer surprises when you get here.

How your first visit works

Five steps from the first call to the start of treatment.

01

Call or book online

Use the booking form or phone the office. A patient coordinator will ask a few basic questions and check your insurance before you come in, so you'll know where your coverage stands ahead of time.

02

Fill out your forms at home

Download the new patient forms from our Online Forms page and complete them before your appointment. Your doctor can read your history in advance, which leaves more of your visit for talking about your pain.

03

Gather your paperwork

Pull together your ID, insurance cards, medication list, prior imaging and any referral. The checklist below has everything. Check-in goes faster when it's all in one folder.

04

Your evaluation

Plan on 45 to 60 minutes with your physician. They'll go over your medical history, ask about your pain in detail, examine you, and look through any prior imaging and records. You leave with a treatment plan written for your situation, not just a prescription.

05

Starting treatment

Depending on your condition and what your insurer requires, a procedure may be scheduled the same day or at a follow-up visit. Our staff handles the prior authorization paperwork for you.

Frequently asked questions

Quick answers for new patients.

What should I bring to my first pain management appointment?

Bring a photo ID, your insurance card (primary and secondary, if you have both), and a list of your current medications with doses. Add any MRI, X-ray or CT images you have, plus notes on past surgeries or treatments. If your plan requires a referral, bring that too. For a workers' compensation or auto injury claim, bring the claim number and your adjuster's contact details.

How long is the first visit?

Plan for 45 to 60 minutes with your physician, and arrive 15 to 20 minutes early so the front desk can check your insurance and collect anything that's missing.

Do I need a referral to see a pain specialist?

It depends on your insurance. PPO plans generally let you book directly with a specialist. HMO plans usually want a referral from your primary care doctor first. When you call, we'll look up your plan and tell you which applies.

Can I fill out the new patient forms before I come in?

Yes, and we'd like you to. You can download the forms from our Online Forms page, complete them at home, and either fax them to the office or bring them with you. Your doctor can then read your history beforehand.

What happens during the first visit?

Your physician reviews your medical history, talks through your symptoms, examines you, and looks at any imaging or records you brought. By the end you'll have a treatment plan built around your condition.

Which insurance plans do you accept?

We accept most major plans, including PPO and HMO plans, as well as workers' compensation and no-fault auto injury claims. Medicare is accepted for qualifying pain management services. For Medicaid, call the office to confirm current participation, since that can change.

Who handles prior authorization?

Our staff does. If your treatment needs approval from your insurer, we prepare and send the paperwork so you don't have to chase it.

Can I be seen the same day?

Sometimes. If your pain is urgent, call the office first thing in the morning and ask about a same-day evaluation. Availability depends on that day's schedule.

How can I pay for my visit?

We take cash, checks, Visa, Mastercard, Discover and American Express for out-of-pocket costs. Payment is due at the time of service. When you schedule, we'll go over your coverage and give you an estimate.
